Here's a link from Emmits fixit Shop.

This was designed for A Heathkit TT-1 tube tester, but it can be changed if you follow the first page instructions.
you don't have to enter test results to get just printed tube numbers, but if you know how to use and modify the calculations you probably use it for any tube tester.
